Introduction
Custom forms let your firm upload its own PDFs (fee agreements, retainers, authorizations, and similar letters), place signature and fill-in fields in DocuSign, and send them from any claim. Templates are shared across the firm. Ready forms appear in Generate Document on every claim.
This feature is entitlement-gated. If you do not see a Forms tab under Settings, ask Counter to enable Custom forms for your firm or user.
Before you begin
- Your account must have Custom forms enabled (Counter Admin firm or personal switch).
- You need a working DocuSign connection for your environment.
- Prefer forms that mainly need a signature plus basic claimant details (name, SSN, contact). Complex questionnaires are better handled with SSA forms or Import from document.
Create a firm form
- Open Settings (gear in the sidebar footer), then open the Forms tab.
- Enter a form name, choose a category, and choose who signs (claimant only, representative only, or both).
- Choose PDF, then Upload & place fields.
- In the DocuSign template editor, place signature tabs and any text fields your form needs, then finish the editor.
- Mark the template Ready when field placement is done.
Draft and archived templates do not appear in Generate Document until they are Ready.
Send a custom form from a claim
- Open the claim in the intake workspace.
- Open Documents → Generate Document.
- Select your firm custom form from the list (alongside SSA document templates).
- Confirm the claimant has an email on the claim when the client must sign.
- Complete any representative signing step in the app if the form requires it. The claimant then receives a DocuSign email to sign.
When the client finishes signing, advocates with Document Signatures email notifications enabled receive an email (see Settings).
Documents drawer
On the claim Documents list you can:
- Track status (for example generated, advocate signed, sent, fully executed).
- Sign or Send when the workflow still needs your action.
- Download the signed PDF when complete.
- Review & sync on a fully signed custom form — opens Import from document with that PDF so you can review extracted fields and merge them into the claim. Always confirm values before applying.
- Delete a custom form instance from the claim (firm custom forms only — not SSA packets).
Tips
- Best for signatures and basic identity fields. Review & sync can miss or misread fields; treat extraction as a draft.
- Keep template names clear so staff can pick the right form in Generate Document.
- Claimant email must be on the claim before client-facing sends.
